As a Hospice and Palliative provider, can cater to adult or occasionally pediatric patients with terminal or chronic care conditions assisting in symptom control, psychosocial support, and Advanced Care Directives discussion. If warranted, may do home visits by appointment. May assist patients needing medical certificates provided that full clinical information and documentation are presented by patients
